ADHD and Back Pain: Why Neurodivergent Adults Often Carry More Physical Tension

The body remains physiologically on alert even when there is nothing to be alert about. There is also the proprioception factor. Many adults with ADHD have a reduced awareness of how they hold their bodies. Sitting in a twisted position for hours, tensing the jaw during concentration, or repeatedly adopting postures that strain the hip flexors are all common. Because proprioception (the sense of where the body is in space) is often muted in ADHD, these habits go unnoticed until they produce pain.

The Sleep Disruption and Pain Connection

ADHD and sleep disorders are closely linked. Delayed sleep phase syndrome is particularly common, meaning ADHD brains tend to switch on late in the evening and struggle to wind down. The result is chronic sleep deprivation.

Poor sleep has a direct impact on pain sensitivity. Research published in Sleep Medicine Reviews has shown that sleep deprivation lowers pain thresholds, increases sensitivity to stimuli, and impairs the body's ability to repair musculoskeletal tissue overnight. The cervical spine and lower back discs are particularly affected, as they rely on overnight rest to rehydrate and recover from the compression of daily activity.

Anxiety, ADHD, and the Tension Response

Anxiety disorders occur in around 50 percent of adults with ADHD. This matters because anxiety has a direct physiological effect on the muscles. The stress response triggers the release of cortisol and adrenaline, both of which prime the muscles for action by increasing tone throughout the body.

If anxiety is a near-constant companion, the muscles of the neck, shoulders, and jaw never fully release. This is known as hypertonicity, and it is a significant driver of tension-type headaches, temporomandibular joint problems, and upper back pain.
