How Stress Accumulates in the Spine: The Science of Somatic Memory

The idea that the body holds onto stress is not new. What has changed in the past thirty years is the emergence of rigorous scientific frameworks for understanding the mechanisms by which psychological and emotional experience produces lasting physical changes in the body. This body of research has transformed the clinical understanding of chronic pain, revealing that stress physiology rather than structural damage underpins many persistent presentations.

The Allostatic Load Model

The concept of allostatic load, developed by McEwen and Stellar in 1993, describes the cumulative physiological cost of the body's continuous adaptation to stressors. When stressors are frequent or sustained, these costs accumulate into chronic physiological strain affecting every body system, including the musculoskeletal system.

High allostatic load is associated with increased pain sensitivity, reduced tissue healing rates, disrupted sleep and impaired immune function. In the spine specifically, chronic cortisol elevation promotes inflammatory processes in disc and joint tissue, maintains muscle tension that compresses spinal structures, and reduces the recovery processes that normally maintain spinal health.

Polyvagal Theory and Spinal Tension

Stephen Porges' Polyvagal Theory describes the hierarchical organisation of the autonomic nervous system. The fight-or-flight state is associated with the characteristic tension pattern of raised shoulders, forward head posture, restricted breathing and truncal bracing. Chronic stress often results in a nervous system that cannot fully return to its relaxed state, producing sustained tension patterns that become embedded in the musculoskeletal system over time.
