Why Does My Back Hurt at Night or First Thing in the Morning?
This is one of the most common questions I hear, and the answer is usually one of the following:
Sleep position - lying in a position that places sustained load on an already-sensitised part of the spine is the most common culprit. Sleeping on your stomach, or curled tightly on your side without support between the knees, can maintain the spine in a compressed or twisted position for hours.
Mattress issues - a mattress that is too soft (allowing the spine to sag) or too firm (creating pressure points) can both contribute to morning back pain.
Inflammatory conditions - morning stiffness and pain that takes more than 30-45 minutes to ease after rising, particularly in younger people, can be associated with inflammatory conditions such as ankylosing spondylitis. These require medical assessment.
Disc problems - intervertebral discs rehydrate overnight as they are no longer under compressive load from body weight. In a disc that is already sensitised or herniated, this increase in disc height can temporarily increase nerve compression.
Muscle spasm - muscles that have been in protective spasm during the day may tighten further during the relative inactivity of sleep, causing stiffness and pain on waking.
Reduced movement - during sleep we move less than when awake, which means stiff spinal joints are held in restricted positions for long periods.
When to Rule Out a Serious Cause
In a small number of cases, night-time back pain can be a sign of something requiring medical attention rather than chiropractic care. These "red flag" symptoms include:
Pain that is constant, progressive, and not affected by position or movement
Significant unintentional weight loss alongside back pain
Night pain accompanied by fever or sweating
Pain in someone with a history of cancer
Back pain in a child or teenager that disturbs sleep

On your back with a pillow under your knees - this flattens the lumbar curve slightly and reduces compression on the facet joints
Stomach sleeping is generally the most problematic position as it forces the neck into sustained rotation and increases the lumbar arch. If you are a confirmed front sleeper, a pillow placed under the abdomen (not the chest) can help reduce the strain on the lower back.
